The City of Columbus is seeking input from residents to guide future community programming and activities. Community members are invited to participate in a survey, which is estimated to take between 10 and 15 minutes to complete.

The survey can be accessed digitally via a QR code that links to a Google Form at https://forms.gle/9kL3dBeohy4quaCg8. For those preferring a paper format, a PDF version is available for download at https://www.cityofcolumbuswi.com/DocumentCenter/View/4570/Family--Community-Activities-Survey-?bidId=. Completed paper surveys can be mailed to 161 N Dickason Blvd., Columbus, WI, or sent by email to ameyers@columbuswi.gov or kbrowskowski@columbuswi.gov.

Participation in the survey provides an opportunity to enter a drawing for gift card giveaways. The deadline for submitting responses is November 2.
